Human review is most effective when escalation thresholds are explicit and tied to workflow state.

Why this matters

Most automation failures are not caused by missing tools. They come from weak process boundaries, missing validation checkpoints, and unclear ownership when behavior drifts. I use this lens to keep systems maintainable under pressure.

Pattern I apply

  • Define confidence or risk thresholds for handoff.
  • Provide reviewers with compact context packets.
  • Track resolution outcomes to tune thresholds.

Failure modes I avoid

  • Routing everything to humans, creating queue overload.
  • No context attached to escalation events.
  • No feedback loop from reviewer decisions.

Practical recommendations

  • Escalate only where risk is material.
  • Instrument approval latency and rework rate.
  • Convert reviewer notes into future automation rules.

What I would test next

  • Add a tiny proof workflow with synthetic inputs and failure injection.
